Insurance Information:

Prior to your appointment: a referral from your primary care doctor will need to be obtained; you can do this by contacting your PCP.

if your insurance requires a referral you can reschedule your appointment or you will be responsible for payment at the time of service.
There is financial aid service at the Central Financial Screening area.
Bring all forms, MRI films and reports, X-Rays, and any medication you are taking.
