Billing Information
It is important that you call your insurance plan to verify that our doctors are on your particular plan. Please give the receptionist your insurance card if you are a new patient or have a change to your insurance coverage.
A registration form will be given to you to verify at the policies. We will, however, provide you with information so that you may bill your cancer policy and receive reimbursement directly to you.
Payment for services are due at the time of service. Copays and patient responsibility balances (coinsurances) are collected at the time that you are checking out from your appointment. Financial arrangements can be made for large blances resulting from surgical services.
A separate billing statement will be sent to you for dermatopathology services relating to the reading of your skin biopsy specimen.
Please contact your Primary Care Physician to obtain a referral authorization, if this is a requirement of your insurance.
Prior authorization is not required by many insurances for procedures that are performed by our dermatologists in the office. Please inform us if your insurance requires that you have prior authorization for procedures done in the doctor's office.
