Evaluation of Infertility

Nearly five million American couples experience infertility. Approximately one in six couples seeks medical help because they are having difficulty in conceiving. Infertility is defined as the inability to conceive after 12 months of having unprotected intercourse (sex without contraception). Just over one million couples seek medical advice regarding infertility on a yearly basis. The average time it takes a couple to conceive is about six months, and around 70% of couples will have achieved a pregnancy by the end of a year of trying. Female infertility can also be divided into primary and secondary infertility. Primary infertility is when you have never conceived; secondary infertility is when you have got pregnant in the past, but have now been trying for at least a year without success.

Anyone can have problems conceiving in at least one in five cases investigated there is absolutely no apparent reason why the couple cannot conceive. The other four in five couples investigated, the problem lies with the man, the woman or the couple together in about equal numbers.

In order to maximize your chances of becoming pregnant, you first need to recognize that you have a problem and know when it is time to seek help.


When should you see an infertility specialist?


  • If you are under age 35 and have been unable to conceive after a year of trying to get pregnant
  • If you are over age 35 and have been unable to conceive after 6 months of trying to get pregnant
  • When you have lost two or more pregnancies to miscarriage
  • If you do not have regular menstrual cycles, or if you have had a prior gynecological problem (for example, endometriosis, tubal pregnancy, pelvic surgery or infections), you should seek assistance sooner.

There are, however, certain factors that place you at a higher risk for infertility factors. These are divided to female, male and couple factors:


��� Female factors:

  • Age (fertility decreases significantly with age)
  • Past damage to, or surgery on, your fallopian tubes
  • Past etopic pregnancy
  • Past sexually transmitted diseases
  • Past surgery to your womb or cervix
  • Not ovulating (due to anorexia nervosa, polycystic ovary syndrome, stress or other causes)
  • Endometriosis
  • Alcohol intake
  • Smoking
  • Obesity
  • Past or current use of certain medications

��� Male factors:

  • Undescended testicles in infancy
  • Past injury to, or operations on, your testicles
  • Past hernia repair
  • Past sexually transmitted disease
  • Problems getting erections or ejaculating
  • Smoking
  • Heavy alcohol consumption
  • Past or current use of certain medications
  • Obesity

��� Couple factors:

  • Not having intercourse enough
  • Not having intercourse at the right time of the cycle
  • Other sexual difficulties or problems
