First Appointment

When you first telephone our office you will be welcomed by one of our staff who will begin gathering information about the nature of your concerns. You will also be asked to obtain medical records from any doctor that you have seen for fertility treatment as well as any records from recent routine care. We recommend that you make a copy of your records to keep for yourself before you send a copy to us.

For your initial visit, you should plan coming 30 minutes before the appointment.

You will also be asked to provide a copy of both partner's insurance cards (front and back) and insurance benefit booklets as well as a copy of each insurance company's third party claim form. These items, including the insurance benefit booklets, can be obtained at your employer's human resources office.
