Insurance Coverage

The Center for Fertility and Reproductive Surgery is contracted with many insurance companies, but coverage for our services depends on your policy. Some policies cover testing only, some cover 100% of treatment, and some policies cover no treatment at all.

Determining your Insurance Coverage

Check your insurance policy booklet to determine your benefits for medical and prescription coverage. There may also be exclusions and limitations listed for infertility benefits including limits on dollar amounts, number of cycles, lifetime limits, and prior authorization requirements.

While having this information in writing is the best way to guarantee coverage, you may also call your insurance company to verify your benefits. The insurance company will likely tell you this is just a confirmation and not a guarantee of benefits. To guarantee coverage you must acquire a predetermination of benefits in writing from your insurance company. Please call our office if you have any further questions about your insurance coverage.

Other Payment Options

If you have little or no coverage for infertility treatments there is another option. We work with American State Bank to help you finance your infertility treatments. They are able to offer our patients an attractive interest rate on loans for these services.
