How Remote Patient Monitoring Keeps You Connected

October 16, 2026

Managing our health often requires a full-time commitment. Taking care of ourselves doesn’t stop when we walk outside the clinic doors.

For patients who are living with a chronic conditions like high blood pressure, diabetes or congestive heart failure, continuing to check in on daily numbers is incredibly important, but those regular check-ins can start to feel overwhelming.

Remote Patient Monitoring provides easy-to-use digital health tools that can help. These resources allow you to connect with and send data to your care team from the comfort of your home, so you can spend your time away from the doctor’s office and stay healthy, safe and supported. We’re providing you with some basic information so that you and your Texas Tech Physicians care team can stay connected and on top of your health needs.

What is Remote Patient Monitoring (PRM) and how does it work?

RPM uses modern digital devices to capture, record and securely transmit your health data directly from your living room.

Depending on your health needs, available devices include:

  • Glucometers
  • Current continuous glucose monitoring device
  • Blood pressure cuffs
  • Weight scales
  • Pulse oximeters
  • Thermometers

Using RPM does not require you to be tech savvy. It’s a seamless process built to support you as a patient. Some of the ways RPM assists you include:

  • Transmitted data: Your devices securely transmit your health data in real time.
  • Monthly check-ins: Our Texas Tech Physicians team reaches out monthly, monitors abnormal readings, surveys your daily activity and symptoms and checks on how you are adhering to your medication.
  • Intervention and care: Our care team provides you with ongoing education and resources and will escalate information to your provider whenever necessary.
  • Continuous review of information: With the help of RPM, Texas Tech Physicians can closely review your data trends and monitor any abnormalities until your readings return to normal or you receive medical attention.

RPM working together with Chronic Care Management

Chronic conditions are any health issues expected to last 12 months or longer. Chronic Care Management (CCM) is for patients who are managing two or more chronic conditions. CCM and RPM work together to reduce your burden as a patient, including lowering the number of visits to the doctor’s office, lowering the cost of healthcare and helping you get the right kind of support when you need it.

CCM provides support that is not face-to-face, including phone calls, secure messaging and personalized care planning that is tailored to your goals, medications and available resources.

The benefits of RPM

Since the ultimate goal of RPM is to put your health first, there are a number of patient benefits to using RPM.

From catching changes in your health before they become serious, to improved delivery of care, to fewer in-person visits and more peace of mind, Texas Tech Physicians is using RPM to support our patients every step of the way.
