Collaborative Care for Better Mental Health
Our Collaborative Care Program brings mental health support into primary care, giving patients greater access to education, connection, and compassionate care while training new clinicians to integrate mental health into everyday healthcare. Together, we’re creating a community where mental health is part of everyone’s care.
What the Team Does
Primary Care Provider (PCP)
In collaborative care, you and your PCP take lead of the care team, your PCP is responsible for overall patient oversight, including diagnosing and managing conditions, developing your treatment plans, and prescribing medications if needed. The PCP works closely with your Behavioral Health Care Manager (BHCM) and a psychiatric consultant (PC), connecting you to the team and coordinating treatment to improve both physical and mental health.
Psychiatric Consultant (PC)
In the collaborative care model, the PC's role is to support PCPs by providing expert guidance on behavioral health conditions without typically seeing you directly or prescribing medication. PCs consult with the care team through regular meetings with the BHCM to assist with diagnoses, treatment planning, and adjusting your care. This ensures that you receive evidence-based, coordinated mental health care within the primary care setting.
Behavioral Health Care Manager (BHCM)
In collaborative care, the BHCM is your central point of contact, coordinating treatment for patients with mental health needs. The BHCM acts as a liaison between you, the PCP, and a PC, providing brief therapy, monitoring progress, and ensuring care plans are followed. This role helps you receive personalized and comprehensive care directly within the primary care setting.
